His name is Brian Carroll, and he's from Sacramento, CA. He took lessons from Paul Gilbert at one point.

just listen to the track 'buckethead' on colonel claypools bucket of bernie brain's album 'the great eyeball in the sky' -- it gives the history of buckethead in goofy song form.
